How It Works

Doxycycline is a tetracycline-class antibiotic that inhibits bacterial protein synthesis. It penetrates bacterial cells via passive diffusion and active transport, then binds to the 30S ribosomal subunit, preventing the addition of amino acids to peptide chains. This action blocks bacterial growth. Mammalian cells are generally unaffected due to the absence of this active transport system.


Indications

Adoxa is used to treat a wide range of bacterial infections, including:

  • Respiratory infections: bronchitis, pneumonia, sinusitis
  • Urinary tract infections: cystitis, urethritis
  • Skin infections: including acne
  • Eye infections
  • Sexually transmitted diseases: gonorrhea, syphilis, chlamydia
  • Rickettsial infections: Q fever, tick-borne fevers
  • Malaria (when resistant to chloroquine)
  • Other infections: cholera, brucellosis, leptospirosis, psittacosis

Dosage

  • Active Ingredient: Doxycycline (as Hyclate)
  • Dose and duration are determined by the physician based on the infection type and patient profile.

Side Effects

  • Very Common/Common:
    • Photosensitivity (rash, redness, sunburn)
    • Headache
    • Nausea, vomiting
    • Diarrhea
    • Rash, hives, flushing
    • Fatigue, dizziness, anxiety
  • Uncommon/Rare:
    • Increased intracranial pressure (severe headache, blurred vision)
    • Inflammation of small blood vessels (Henoch-Schönlein purpura)
    • Jarisch-Herxheimer reaction (fever, chills, rash in spirochetal infections)
    • Porphyria, pancreatitis, thyroid discoloration
    • Blood disorders (anemia, leukopenia, etc.)

Drug Interactions

  • Antibiotics/Antivirals: Penicillin, rifampicin
  • CYP inducers: Phenobarbital, phenytoin, carbamazepine, primidone
  • Blood thinners: Warfarin
  • Immunosuppressants: Ciclosporin
  • Retinoids: Oral isotretinoin (↑ risk of intracranial hypertension)
  • Antacids or supplements containing: calcium, magnesium, iron, bismuth, zinc (reduce absorption)
  • Methotrexate, methoxyflurane, quinapril, and oral contraceptives

When Not to Use

  • Hypersensitivity to doxycycline or any tetracycline-class drug
  • Severe liver dysfunction
  • Infants and children under 8 years (due to tooth discoloration and growth inhibition)

Precautions

  • Avoid combining with oral retinoids due to risk of intracranial hypertension
  • Caution in patients with hepatic impairment or esophageal disorders

Warnings

Warning 1:
Avoid excessive exposure to sunlight or UV rays — may cause severe sunburn (photosensitivity).

Warning 2:
Use with caution in myasthenia gravis patients — may worsen muscle weakness.

Warning 3:
May exacerbate symptoms of systemic lupus erythematosus (SLE) — report any facial rash, joint pain, or fever to a doctor.
