Mechanism of Action
Candesartan is an angiotensin II receptor blocker (ARB).
•    Blocks angiotensin II from binding to AT1 receptors in tissues such as vascular smooth muscle and the adrenal gland
•    Reduces vasoconstriction, aldosterone secretion, cardiac stimulation, and sodium reabsorption
•    Does not depend on the synthesis pathway of angiotensin II for its effect

Indications
•    Treatment of hypertension in adults and children aged 1 to <17 years
•    Can be used alone or in combination with other antihypertensives

Side Effects
General:
•    Fatigue (asthenia), fever
Nervous System:
•    Paresthesia, vertigo
Gastrointestinal:
•    Indigestion (dyspepsia), gastroenteritis
Cardiac:
•    Palpitations, tachycardia
Metabolic:
•    Hyperglycemia, hypertriglyceridemia, hyperuricemia, increased creatine phosphokinase
Musculoskeletal:
•    Myalgia (muscle pain)

Drug Interactions
Caution when used with:
•    Glyburide, Nifedipine, Digoxin, Warfarin, Hydrochlorothiazide, Oral contraceptives
•    ACE inhibitors (e.g., Enalapril)
•    Potassium-sparing diuretics, Potassium supplements, Heparin

Do not use in:
•    Patients hypersensitive to candesartan or any of its components

Precautions
•    In patients with ischemic heart or cerebrovascular disease, an excessive drop in BP could cause myocardial infarction or stroke

Warnings
1.    Hypotension risk in heart failure or dehydrated hypertensive patients—monitor BP closely, especially during treatment initiation
2.    May cause elevated blood urea/serum creatinine in patients with renal artery stenosis
3.    Patients on dialysis may be more sensitive to AT1-blockade—careful BP titration and monitoring required
