Mechanism of Action:
Cefixime, a third-generation cephalosporin, works by binding to penicillin-binding proteins
(PBPs) in the bacterial cell wall. This inhibits the final stage of peptidoglycan synthesis,
compromising cell wall integrity and leading to bacterial cell death.
Indications:
Cefixime is effective against infections caused by susceptible microorganisms, including:
 Uncomplicated urinary tract infections
 Otitis media
 Pharyngitis and tonsillitis
 Acute bronchitis and acute exacerbations of chronic bronchitis
 Uncomplicated gonorrhea (cervical/urethral)
Dosage Form:
Suspension – 200 mg/5 mL
Total Volume: 30 mL

Side Effects:
 Gastrointestinal: Diarrhea (discontinue if severe), nausea, vomiting, abdominal pain,
dyspepsia, stool discoloration
 Neurological: Headache, dizziness, seizures
 Hepatic: Transient elevation of liver enzymes (transaminases, alkaline phosphatase),
jaundice
 Pancreatic: Elevated serum amylase

Use caution with concurrent use of:
 Anticoagulants
 Carbamazepine
 Probenecid
Contraindications:
Do not use in:
 Patients with known hypersensitivity to cefixime
 Patients allergic to other cephalosporin antibiotics
Precautions:
 Use cautiously in penicillin-sensitive patients due to partial cross-reactivity
 Monitor closely in patients with renal impairment
 Use with caution in patients with a history of gastrointestinal diseases, especially colitis
Warnings:
1. Renal Impairment: Adjust dosage or monitor as needed
2. Colon Flora Disruption: May lead to clostridial overgrowth, including C. difficile
3. Gastrointestinal Risk: Increased caution in patients with a history of colitis or chronic
