Mechanism of Action:
Cefixime, a third-generation cephalosporin, works by binding to penicillin-binding proteins
(PBPs) in the bacterial cell wall. This action inhibits the final stage of cell wall synthesis,
leading to bacterial lysis and death.
Indications:
Cefixime is used to treat infections caused by susceptible strains of bacteria, including:
 Uncomplicated urinary tract infections (UTIs)
 Otitis media
 Pharyngitis and tonsillitis
 Acute bronchitis and acute exacerbations of chronic bronchitis
 Uncomplicated gonorrhea (cervical/urethral)
Dosage Form:
Suspension – 200 mg/5 mL (Double Strength)
Total Volume: 30 mL
Side Effects:
 Gastrointestinal: Diarrhea (stop if severe), nausea, vomiting, abdominal pain, dyspepsia,
stool discoloration
 Neurological: Headache, dizziness, seizures
 Liver: Transient elevation in liver enzymes (transaminases, alkaline phosphatase),
jaundice
 Pancreas: Elevated serum amylase
Drug Interactions:
Use with caution when combined with:
 Anticoagulants
 Carbamazepine
 Probenecid
Contraindications:
 Known hypersensitivity to cefixime or other cephalosporins
Precautions:
 Exercise caution in patients with penicillin allergies due to cross-allergenicity
 Use with care in patients with renal impairment
 Avoid or closely monitor in patients with gastrointestinal diseases, especially colitis
Warnings:
1. Renal Impairment: Monitor kidney function and adjust dosage accordingly
2. Microflora Disturbance: Broad-spectrum antibiotics may cause Clostridium difficile
overgrowth
3. GI Risk: Increased risk of colitis in susceptible individuals
