How It Works
Celecoxib is a COX-2 selective nonsteroidal anti-inflammatory drug (NSAID).
 It reduces pain, inflammation, and fever by inhibiting cyclooxygenase-2 (COX-2), an
enzyme involved in prostaglandin synthesis.
 At therapeutic doses, it does not significantly inhibit COX-1, minimizing gastric side
effects compared to traditional NSAIDs.
Usage and Safety
 Dosage: As prescribed by a physician; contains Celecoxib.
Common Side Effects:
 Gastrointestinal: Abdominal pain, diarrhea, dyspepsia, flatulence, nausea
 Nervous System: Dizziness, headache
 Respiratory: Sinusitis, pharyngitis, rhinitis
 Others: Back pain, insomnia, rash
Less Common to Rare Side Effects:
 GI: Constipation, gastritis, reflux, bleeding
 Cardio: Hypertension, angina, heart attack

Drug Interactions
Use with caution or avoid combining with:
 ACE inhibitors / ARBs (e.g., for blood pressure)
 Diuretics (e.g., furosemide, thiazides)
 Aspirin and other NSAIDs
 Fluconazole, Warfarin (increased bleeding risk)
 Lithium (can increase lithium levels)
 Antacids, glucocorticoids
Indications
Celecoxib is used for:
1. Osteoarthritis – pain and inflammation
2. Rheumatoid arthritis (adults)
3. Ankylosing spondylitis
4. Juvenile rheumatoid arthritis (age ≥2 years)
5. Acute pain in adults, including post-operative pain
Contraindications (When Not to Use)
Do not use in patients with:
 Allergy to celecoxib, sulfonamides, or NSAIDs
 History of asthma or allergic reactions to aspirin/NSAIDs
 Severe liver or kidney impairment
 History of heart attack, stroke, heart failure, or GI bleeding
 Use before or after coronary artery bypass (CABG) surgery
 Peptic ulcer, inflammatory bowel disease, or bleeding disorders
Precautions
 Do not substitute for corticosteroids in steroid-dependent patients
 Taper corticosteroids gradually to avoid adrenal insufficiency if discontinuing
 Caution in patients with fluid retention, hypertension, or cardiac disease
Warnings
1. Edema & fluid retention can worsen heart failure or hypertension.
2. Celecoxib may raise blood pressure or worsen existing hypertension.
3. Liver toxicity: Monitor liver function; stop if signs of liver damage appear (e.g.,
jaundice, rash, eosinophilia).
