Mechanism of Action
 Celecoxib is a COX-2 selective nonsteroidal anti-inflammatory drug (NSAID).
 It inhibits prostaglandin synthesis, primarily by blocking COX-2 enzyme.
 At therapeutic doses, it does not inhibit COX-1, reducing gastrointestinal side effects.
Indications
1. Osteoarthritis
2. Rheumatoid arthritis (Adults)
3. Ankylosing spondylitis
4. Juvenile rheumatoid arthritis (≥2 years)
5. Acute pain in adults, including postoperative pain
Dosage
 Active Ingredient: Celecoxib
 Dosage varies based on condition and patient profile. Follow physician’s prescription.
Side Effects
Common:
 Gastrointestinal: Abdominal pain, diarrhea, flatulence, nausea, dyspepsia
 Neurological: Dizziness, headache
 Respiratory: Pharyngitis, sinusitis, upper respiratory infections
 Others: Rash, insomnia, back pain
Less Common:
 GI: Constipation, reflux, vomiting
 Cardiac: Palpitations, angina, hypertension
 CNS: Vertigo, paresthesia, leg cramps
 Psychiatric: Anxiety, somnolence, depression
 Respiratory: Bronchitis, coughing, pneumonia
Very Rare (Serious):
 Heart failure, stroke, pulmonary embolism
 Severe skin reactions: Stevens-Johnson syndrome, toxic epidermal necrolysis
 Hepatitis, pancreatitis, renal failure, aplastic anemia, agranulocytosis
 Anaphylactoid reactions, angioedema
Contraindications (Do NOT Use If):
 Allergic to celecoxib, sulfonamides, or NSAIDs
 History of asthma, urticaria, or anaphylaxis from NSAIDs
 Severe liver or kidney impairment (Child-Pugh Class C or CrCl <30 mL/min)
 Active GI bleeding or ulcers
 History of myocardial infarction, stroke, or heart failure
 During CABG surgery (perioperative use)
 Inflammatory bowel disease
Drug Interactions
Use caution with:
 ACE inhibitors, ARBs (e.g., for blood pressure)
 Diuretics: Furosemide, thiazides
 Aspirin, Warfarin, Glucocorticoids (↑ GI/bleeding risk)
 Lithium, Fluconazole, Antacids
Precautions
 Not a corticosteroid substitute. Avoid abrupt corticosteroid withdrawal.
 Taper corticosteroids gradually under supervision.
Warnings
1. May cause fluid retention or worsen heart failure and hypertension
2. Risk of cardiovascular events (MI, stroke) — monitor blood pressure
3. Liver monitoring recommended if signs of liver dysfunction occur
