Used For

Mechanism of Action
Clarithromycin binds to the 50S ribosomal subunit of susceptible bacteria, inhibiting protein
synthesis and preventing bacterial replication.
Dosage
 Active Ingredient: Clarithromycin
 Dosage should be prescribed based on age, weight, infection type, and renal function
by a qualified healthcare provider.
Side Effects

Serious / Rare:
 Allergic reactions: urticaria, mild rash, anaphylaxis

 GI: anorexia, vomiting, pancreatitis
 Tooth/tongue discoloration (reversible)
 Dizziness, hearing loss (usually reversible)
 Altered sense of smell and taste
 CNS symptoms: anxiety, hallucinations, insomnia, depression, psychosis
 Cardiac: QT prolongation, ventricular arrhythmias, torsades de pointes
 Blood disorders: thrombocytopenia, leukopenia, neutropenia
 Rare: interstitial nephritis
Drug Interactions
Avoid Concomitant Use With:
 Cisapride, Pimozide, Astemizole, Terfenadine – Risk of life-threatening arrhythmias
 Ergot alkaloids (e.g., ergotamine, dihydroergotamine) – Risk of ergot toxicity
 Oral Midazolam – Risk of excessive sedation
 HMG-CoA Reductase Inhibitors (statins) – Risk of muscle toxicity
Contraindications
 Known hypersensitivity to clarithromycin or macrolide antibiotics
 Concomitant administration with the drugs listed above under Drug Interactions
Precautions
 Pregnancy: Use only if clearly needed, especially in the first trimester
 Long-term Use: May result in overgrowth of non-susceptible organisms (bacterial or
fungal)
 Monitor for signs of superinfection
Warnings
1. Renal Caution:
Use cautiously in patients with severe renal insufficiency
2. Hepatic Dysfunction:
o Clarithromycin may cause increased liver enzymes and hepatocellular or
cholestatic hepatitis
o Generally reversible but requires monitoring
3. Severe Hepatic Failure:
o Rare, potentially fatal cases have been reported
o Discontinue immediately if symptoms such as anorexia, jaundice, dark urine,
pruritus, or abdominal tenderness occur
