2. Mechanism of Action
 Fluoxetine:
A selective serotonin reuptake inhibitor (SSRI). Inhibits CNS neuronal reuptake of
serotonin (5-HT) with minimal effect on norepinephrine or dopamine. Does not bind to
alpha-adrenergic, histaminergic, or cholinergic receptors.
 Olanzapine:
Atypical antipsychotic. Potent antagonist of dopamine (D1–D3), serotonin (5-HT2A, 5-
HT2C), histamine H1, and alpha1-adrenergic receptors. Moderate antagonism at
muscarinic M1–M5 and 5-HT3 receptors; also binds GABA-A, benzodiazepine, and βadrenergic receptors.
3. Dosage
 Contains fixed-dose combination of Olanzapine + Fluoxetine
 Dosage varies based on clinical condition and patient tolerance—must be prescribed by
a physician
4. Side Effects
 Common to Severe Effects Include:
o Neurological: Chills, neck rigidity, QT prolongation
o Gastrointestinal: Diarrhea, nausea, vomiting, peptic ulcer, GI bleeding, intestinal
obstruction
o Hepatic & Renal: Liver fatty deposit, bilirubinemia, pancreatitis, elevated
creatinine
o Hematologic: Anemia, leukopenia, thrombocytopenia, purpura
o Musculoskeletal: Osteoporosis
o Other: Photosensitivity, weight loss, ecchymosis, generalized edema, gout
5. Drug Interactions
Avoid or use caution with:
 MAOIs (contraindicated)
 CNS-active agents (e.g., benzodiazepines, antipsychotics, SSRIs)
 Serotonergic agents, triptans, tryptophan
 Anticoagulants/NSAIDs (e.g., aspirin, warfarin)
 CYP1A2 inducers/inhibitors
 Tricyclic antidepressants, antiepileptics, antihypertensives, lithium, valproate,
theophylline
6. Indications
 Depressive Episodes in Bipolar I Disorder (Adults)
 Treatment-Resistant Depression (Adults): For MDD that has not responded to two
separate antidepressants of adequate dose and duration
7. Contraindications (When NOT to Use)
 Concurrent use of MAOIs, Pimozide, or Thioridazine
 Known hypersensitivity to Olanzapine, Fluoxetine, or any tablet component
8. Precautions
 Patients on antidepressants should be closely monitored for:
o Clinical worsening
o Emergent suicidality
o Behavioral changes (especially during first few months or dose changes)
9. Warnings
Warning 1:
In cases of worsening depression or new suicidality, consider modifying or discontinuing
therapy.
Warning 2:
Patients with MDD may experience worsening mood or emergence of suicidal thoughts—this
risk exists regardless of medication use.
Warning 3:
Although the suicidality risk with long-term use is unclear, maintenance therapy has shown
benefit in delaying recurrence in adults.
