Therapeutic Use
Indicated for the Treatment of:
 Gastroesophageal Reflux Disease (GERD):
o Treatment of erosive reflux esophagitis
o Long-term management of healed esophagitis to prevent relapse
o Symptomatic treatment of GERD
Mechanism of Action
 Omeprazole is a proton pump inhibitor (PPI) that reduces gastric acid secretion by
irreversibly blocking the H⁺/K⁺ ATPase enzyme system (the proton pump) in gastric
parietal cells.
 Sodium Bicarbonate:
o Acts as an antacid, quickly neutralizing stomach acid.
o Protects omeprazole from gastric degradation, allowing for faster and more
effective absorption.
 The combined effect is a rapid onset of acid suppression and prolonged relief from
acid-related symptoms.

 Typical formulations: Capsules or powder for oral suspension containing Omeprazole
+ Sodium Bicarbonate
Side Effects
Common:
 CNS: Headache
 GI: Diarrhea, constipation, abdominal pain, nausea, vomiting, flatulence
Uncommon:
 CNS: Dizziness, paraesthesia, drowsiness, insomnia, vertigo
 Hepatic: Elevated liver enzymes
 Skin: Rash, pruritus, urticaria
 General: Malaise
Rare:
 CNS (especially in severely ill patients): Mental confusion, agitation, aggression,
depression, hallucinations
 Endocrine: Gynecomastia
Drug Interactions
May interact with:
 Antifungals: Ketoconazole, itraconazole, posaconazole
 CNS drugs: Diazepam, citalopram, imipramine, clomipramine, phenytoin
 Antivirals: Atazanavir, saquinavir, nelfinavir (contraindicated)
 Others: Clopidogrel, digoxin, erlotinib, tacrolimus, methotrexate, clarithromycin
 Enzyme inducers/inhibitors: Rifampicin, St. John’s Wort (CYP2C19 & CYP3A4)
Contraindications
 Known hypersensitivity to omeprazole, sodium bicarbonate, or substituted
benzimidazoles
 Concurrent use with nelfinavir
Precautions
 A positive response to treatment does not exclude gastric cancer; further evaluation
may be needed if symptoms persist or worsen.
Warnings
1. Atrophic Gastritis: Observed in some long-term omeprazole users; monitor as needed.
2. Sodium Bicarbonate Use: Caution in patients with:
o Bartter’s syndrome
o Hypokalemia
o Respiratory alkalosis
o Acid-base disturbances
o Risk of milk-alkali syndrome when taken with calcium or milk long-term
3. Sodium Content Alert: Contains sodium bicarbonate; use cautiously in patients on
sodium-restricted diets.
