How It Works:
Tiotropium is a long-acting antimuscarinic (LAMA) agent. It binds to muscarinic M3
receptors in the airway smooth muscle and inhibits bronchoconstriction, resulting in
bronchodilation.
 Onset of action: Within 30 minutes
 Duration: Over 24 hours
 The extended effect is due to slow dissociation from M3 receptors
Dosage:
Tiotropium Bromide
Administered via Dry Powder Inhaler (DPI), once daily as prescribed.
Side Effects:
 Common: Dry mouth
 Uncommon: Dizziness, blurred vision, headache, taste disturbances, atrial fibrillation,
pharyngitis, cough, GERD, constipation, oral thrush, rash, urinary retention
 Rare: Insomnia, glaucoma, increased intraocular pressure, tachycardia, bronchospasm,
nausea, sinusitis, intestinal obstruction, urticaria, hypersensitivity, urinary tract infection
 Unknown Frequency: Anaphylaxis, dry skin, skin ulcer, joint swelling, dental caries,
dehydration
Drug Interactions:
 Can be safely used with:
o Short/long-acting β2-agonists (e.g., salbutamol, formoterol)
o Methylxanthines (e.g., theophylline)
o Corticosteroids
 Avoid co-administration with:
o Other anticholinergic medications, due to risk of additive anticholinergic effects
Indication:
 Maintenance treatment of bronchospasm associated with COPD
 Reduces exacerbation frequency in COPD patients
When Not to Use:
 Hypersensitivity to:
o Tiotropium Bromide
o Atropine derivatives (e.g., ipratropium, oxitropium)
o Any excipients in the formulation
Precautions:
 Not for acute symptom relief or rescue therapy
 Avoid use in patients with narrow-angle glaucoma, urinary retention, or prostatic
hyperplasia unless monitored closely
 Use cautiously in moderate to severe renal impairment (CrCl < 60 mL/min)
Warnings:
1. Hypersensitivity Reactions:
 May include rash, urticaria, swelling of lips/tongue/throat, anaphylaxis
 Discontinue immediately if observed
2. Paradoxical Bronchospasm:
 Rare but serious; requires immediate treatment with a short-acting bronchodilator
 Stop tiotropium if this occurs
3. Renal Impairment:
 Monitor patients for anticholinergic side effects if renal function is reduced
