How It Works:
Tiotropium is a long-acting antimuscarinic (anticholinergic) agent that binds to muscarinic
receptors (M1 to M5) in the airway smooth muscles. Its primary therapeutic action occurs
through M3 receptor inhibition, which leads to sustained bronchodilation. The
bronchodilatory effect lasts over 24 hours due to slow dissociation from M3 receptors,
providing long-term symptom relief in COPD patients.

Side Effects:
 Common:
o Dry mouth
 Uncommon:
o Dizziness, headache, altered taste, blurred vision, cough, pharyngitis, dysphonia,

o Insomnia, glaucoma, increased intraocular pressure, arrhythmias, epistaxis,
bronchospasm, nausea, rash, hypersensitivity, angioedema, UTI

Drug Interactions:
 Safe with:
o Short-acting and long-acting β2-agonists, methylxanthines, corticosteroids
 Caution with:
o Other anticholinergic drugs — avoid co-administration due to risk of increased
anticholinergic adverse effects
Indications:
Tiotropium is indicated for:
 Long-term, once-daily maintenance of bronchospasm in COPD
 Prevention of exacerbations in COPD patients
When Not to Use:
 Contraindicated in:
o Patients with known hypersensitivity to:
 Tiotropium
 Atropine or its derivatives (e.g., ipratropium, oxitropium)
 Any excipients in the formulation
Precautions:
 Not to be used for acute bronchospasm
 Intended for maintenance therapy only, not for emergency relief
Warnings:
1. May cause severe allergic reactions (e.g., urticaria, angioedema, bronchospasm). Discontinue
immediately if such reactions occur.
2. Paradoxical bronchospasm can occur. If so, treat with a short-acting beta2-agonist and stop
tiotropium.
3. Use with caution in patients with renal impairment (CrCl < 60 mL/min); monitor for
anticholinergic side effects.
