Used For:
Treatment and prevention of hemorrhage due to increased fibrinolysis or fibrinogenolysis.
How It Works:
Tranexamic acid is an antifibrinolytic agent. It competitively inhibits the activation of
plasminogen by binding to its kringle domain, thereby reducing its conversion into plasmin, an
enzyme responsible for the breakdown of fibrin clots. It also directly inhibits plasmin activity,
but requires higher doses for this effect. The net result is stabilization of blood clots and
reduced bleeding.
Dosage:
Tranexamic Acid (dose as directed by a physician depending on the indication and patient
condition)
Side Effects:
 Common:
o Hypotension (especially after rapid IV administration)
o Hypersensitivity skin reactions
 Less Common:
o Thrombotic complications (usually due to inappropriate use)
o Transient visual disturbances (especially color vision)
Drug Interactions:
 Estrogens (e.g., oral contraceptives) – May increase the risk of thrombosis
 Thrombolytic agents – Opposing mechanism, may reduce efficacy
Indications:
Tranexamic acid is indicated for:
 Treatment of hemorrhage or risk of hemorrhage due to excess fibrinolysis or
fibrinogenolysis
 Management of acute bleeding where hyperfibrinolysis is a significant factor
When Not to Use:
Contraindicated in patients with:
 Hypersensitivity to tranexamic acid or formulation excipients
 Active thromboembolic disease (e.g., DVT, pulmonary embolism, cerebral thrombosis)
 Consumption coagulopathy (e.g., DIC), unless fibrinolysis predominates with severe
bleeding
Precautions:
 Evaluate risk factors for thromboembolism before initiating therapy
 In patients with a history or family history of thrombotic events, use only if strongly
indicated and under strict medical supervision
Warnings:
1. Use with caution in patients taking oral contraceptives due to an increased risk of
thrombosis.
2. Do not use in patients with undiagnosed irregular menstrual bleeding until a proper
diagnosis is made.
3. Convulsions have been reported with tranexamic acid use, especially in high-dose or
intravenous treatment settings.
