Tranexamic Acid
Used For:
Management and prevention of hemorrhage due to excessive fibrinolysis or fibrinogenolysis.
How It Works:
Tranexamic acid is an antifibrinolytic agent. It works by competitively inhibiting the
activation of plasminogen (by binding to the kringle domain), thereby reducing its conversion to
plasmin, an enzyme responsible for breaking down fibrin clots and other plasma proteins. At
higher doses, it also directly inhibits plasmin activity. This results in stabilization of clots and
reduced bleeding.
Dosage:
As directed by a physician, based on the condition and clinical need. Contains Tranexamic
Acid.
Side Effects:
 Common:
o Low blood pressure (especially with rapid IV administration)
o Skin hypersensitivity reactions (rash, itching)
 Less Common:
o Thrombotic events (typically due to misuse or inappropriate indication)
o Visual disturbances (e.g. color vision changes)
Drug Interactions:
 Estrogens (e.g., oral contraceptives) – May increase thrombotic risk
 Thrombolytic agents – Antagonistic effect with Tranexamic acid
Indications:
Tranexamic acid is indicated for:
 Treatment and prevention of hemorrhage or risk of hemorrhage in patients with
increased fibrinolysis or fibrinogenolysis
Contraindications (When Not to Use):
 Hypersensitivity to Tranexamic acid or formulation components
 Active thromboembolic disorders such as:
o Deep vein thrombosis (DVT)
o Pulmonary embolism
o Cerebral thrombosis
 In consumption coagulopathy unless there is predominant hyperfibrinolysis with
severe bleeding
Precautions:
 Assess risk factors for thrombosis before use
 In patients with personal or family history of thromboembolic disease, use only if
strongly indicated and under close medical supervision
Warnings:
1. Use with caution in patients on oral contraceptives due to increased risk of thrombosis.
2. Avoid use in patients with unexplained irregular menstrual bleeding until the cause is
clearly diagnosed.
3. Seizures have been reported, particularly with intravenous administration or high doses.
Pregnancy Category:
Consult your physician before using during pregnancy or breastfeeding.
