How It Works:
Cephradine is a first-generation cephalosporin antibiotic that:
 Inhibits the final transpeptidation step of bacterial cell wall synthesis
 Binds to penicillin-binding proteins (PBPs)
 Causes cell wall lysis and bacterial cell death

Side Effects:
 Gastrointestinal:
o Glossitis, nausea, vomiting, diarrhea, abdominal pain, pseudomembranous colitis
 Hypersensitivity:
o Rash, urticaria, erythema, pruritus, drug fever, joint pain
 Hematologic:
o Eosinophilia, leukopenia, neutropenia (usually mild and transient)
 Other:
o Headache, dizziness, dyspnea, paresthesia, candidiasis, vaginitis
Drug Interactions:
 Loop diuretics – may increase nephrotoxicity
 Probenecid – may increase cephradine serum levels
 Aminoglycosides – increased risk of nephrotoxicity
 Anticoagulants – monitor INR when used concomitantly
Indication:
Treatment of infections caused by susceptible organisms in the:
 Respiratory tract
 Urinary tract
 Skin and soft tissues
 Gastrointestinal tract
When Not to Use:
 Known hypersensitivity to cephradine or other cephalosporins
 Known allergic reaction to any ingredient in the formulation
Precautions:
 Use with caution in patients allergic to penicillins due to potential cross-reactivity
 Adjust dosage in renal impairment
Warnings:
1. Renal Impairment:
Use caution in patients with kidney disease; dosage adjustment may be necessary.
2. False Urine Glucose Tests:
May cause false-positive glucose results with Benedict's or Fehling’s solution and Clinitest
tablets.
3. Superinfection Risk:
Prolonged use may lead to overgrowth of non-susceptible organisms, including fungi.
