Generic Name:
Fluconazole
Used For:
Fungal Infections, especially caused by Candida species.
How It Works:
Fluconazole is a triazole antifungal agent that inhibits fungal cytochrome P-450-mediated 14
alpha-lanosterol demethylation, a critical step in fungal ergosterol biosynthesis.
 This disruption causes accumulation of toxic sterols and loss of ergosterol, compromising
the fungal cell membrane and leading to antifungal effects.
 Fluconazole selectively targets fungal cytochrome P-450 enzymes more than mammalian
ones, reducing toxicity.
Dosage:
Fluconazole (as prescribed by a healthcare professional)
Side Effects:
 Common (up to 1 in 10):
Headache, stomach discomfort, diarrhea, nausea, vomiting, increased liver enzymes, rash
 Uncommon (up to 1 in 100):
o Reduction in red blood cells (anemia) causing pallor, weakness, breathlessness
o Decreased appetite, insomnia, drowsiness
o Fits (seizures), dizziness, vertigo, tingling or numbness, altered taste
o Constipation, indigestion, flatulence, dry mouth
o Muscle pain, liver damage, jaundice (yellowing skin/eyes)
o Skin reactions (hives, itching, increased sweating)
o Fatigue, malaise, fever
Drug Interactions:
Significant interactions with many drugs including but not limited to:
 Rifampicin, rifabutin
 Alfentanil, fentanyl
 Amitriptyline, nortriptyline
 Amphotericin B, voriconazole
 Warfarin
 Benzodiazepines (midazolam, triazolam)
 Carbamazepine, phenytoin
 Calcium channel blockers (nifedipine, verapamil, etc.)
 Immunosuppressants (ciclosporin, tacrolimus)
 Chemotherapy agents (cyclophosphamide, vinca alkaloids)
 Statins (atorvastatin, simvastatin)
 NSAIDs (celecoxib, ibuprofen, diclofenac)
 Oral contraceptives
 Prednisone (steroid)
 HIV drugs (zidovudine, saquinavir)
 Antidiabetics (chlorpropamide, glibenclamide)
 Theophylline, tofacitinib, tolvaptan
 Vitamin A supplements
 Amiodarone, hydrochlorothiazide, ibrutinib

Indication:
 Treatment of fungal infections caused by Candida and other fungi
 Prevention of candidal infections
When Not to Use:
 Allergy to fluconazole or other antifungal medicines
 Use of astemizole or terfenadine (antihistamines)
 Use of cisapride (for stomach issues)
Symptoms of allergy may include itching, skin redness, difficulty breathing.
Precautions:
 Serious skin reactions such as DRESS (drug reaction with eosinophilia and systemic
symptoms) may occur.
 Stop medicine and seek medical help if you notice rash, swelling, fever, or other signs of
serious skin reaction.
Warnings:
1. Use cautiously in patients with renal dysfunction.
2. Rare but serious hepatic toxicity has been reported, sometimes fatal, especially in those
with severe illnesses. Usually reversible upon stopping the drug.
3. Use caution in patients with proarrhythmic conditions. Avoid coadministration with
other drugs that prolong QT interval and are metabolized by CYP3A4.
